Medical Records

To obtain a copy of your medical records, radiology films and itemized bills, call or visit the Release of Information Center in the Plaza Diagnostic Center, at 7777 Hennessy Boulevard, Suite 113. The number is 5-8541 from your hospital room, or (225) 765-8541 from outside of the hospital. The Information Management (Medical Records) department maintains patients' medical records.
