Care for the Elderly

We will test and develop new, innovative geriatric models and processes across a broad spectrum of services.

Advanced Care Planning – Our Senior Services Division is leading an effort to support our patients and families with end-of-life decisions. This is being accomplished by training team members to participate in the difficult conversations about advanced directives and living wills. We are extending this effort to other providers such as nursing homes, home health, hospice companies and faith-based organizations.

LaPOST – Working with the Louisiana Health Care Quality Forum, we are leading the Louisiana Physician Orders for Scope of Treatment (LaPOST) initiative. This is an official document that translates a patient’s end-of-life wishes and goals of care into physician orders that transfer across healthcare settings.

PACE (Programs of All Inclusive Care for the Elderly) – This unique program serves people who would otherwise need nursing home care. One of only 94 such programs in the nation, this model of care for the elderly results in strong clinical outcomes, as well as a reduction of both hospitalization and re-hospitalization of its participants. 

Nursing Care Education – Our Lady of the Lake College is leading an effort to develop nurse leaders who will provide care for older patients. OLOL College has a goal to be a Center of Gerontological Excellence, positioning our system as a national model for the care of seniors.
