About Our Program
The Cardiopulmonary Rehabilitation program at Our Lady of the Lake Hearth Center is designed to help you recover faster from your heart event and return more quickly to a full and productive life. An individually designed program involving supervised exercise, education, lifestyle counseling and risk factor modification may help you resume your normal work and leisure activities with greater physical capacity. The key elements of cardiac rehabilitation involve:
Exercise
- An individual program of physical activity is tailored to your abilities and personal needs, carried out in a group setting
- Learning how to exercise safely and effectively, while monitored and supervised by our professional staff
- Strengthening your muscles and improving stamina
- Exercise classes are offered Monday - Friday. Please call for specific times.
Education
- Information about your heart problem, its causes, treatments and ways to to help with lifestyle changes is offered in Heart-Healthy Lifestyle Education Series classes
- Learn how to take more responsibility for your health in order to prevent or minimize any future heart problems
- A variety of Cardiac Education Programs are offered throughout the year. Please check our Calendar of Events for class offerings.
Counseling and Behavior Change
- Developing strategies that will successfully lower your risk of recurring heart disease
- Learning specific skills to help you stop unhealthy behaviors, such as smoking
- Beginning healthy behaviors, such as eating a heart- healthy diet and stress reduction
- Gathering insight into emotional and psychological health after a cardiac event.
Safe and Effective Recovery
According to the Clinical Practice Guidelines developed by the Agency for Health Care Policy and Research of the U.S. Department of Health and Human Services, cardiac rehabilitation is considered to be an essential component of the management of patients who have experienced a cardiac event.
Cardiac rehabilitation has proven to be a safe and effective method to help patients recover from heart difficulties, reduce future hospital admissions and improve patient recovery.
You can benefit from cardiac rehabilitation if you have experienced any of the following:
- Cardiac Chest Pain
- Coronary Artery Bypass Surgery
- Myocardial Infarction (Heart Attack)
- Angina
- Congestive Heart Failure
- Valve Replacement
- Balloon Angioplasty (PTCA)

Your Cardiac Rehabilitation Team
Your cardiac rehabilitation team always begins with your personal physician. Other team members from Our Lady of the Lake Heart Center include:
- Registered Nurses
- Clinical Exercise Physiologists
- Registered Dietitians
- Licensed Social Workers
Each member of your cardiac rehabilitation team will be an important resource for your recovery and for preventative management.
How to Begin Your Cardiac Rehabilitation
Your physician must first refer you to the Cardiac Rehabilitation Center. You may call us directly or your physician may notify us and provide a referral order. We will then contact you to set an orientation appointment and assist you with your insurance arrangements. Medicare and most insurance carriers usually cover at least 80 percent of the cost of services, but it is important to verify coverage.
The phases of cardiac rehabilitation include:
Phase I: In the Hospital
This includes low-level activity and initial education following a cardiac event that requires close monitoring.
Phase II: Outpatient
This centers around EKG-monitored, supervised activity according to an individual exercise prescription, and in-depth education about recovery and prevention with an emphasis on lowering the factors that cause heart disease. Treatment is guided by patient goals, needs and abilities.
Phase III: Maintenance
This involves minimal supervision of exercise with emphasis on increased independence. Individuals may continue to exercise with the Cardiac Rehabilitation Center until they are confident enough to begin an unsupervised exercise program.
Phase IV: Independence
This phase continues with unsupervised, independent exercise on your own. Our Lady of the Lake Cardiac Rehabilitation Center remains a resource for information and advice.

Certification
The cardiac rehabilitation program at Our Lady of the Lake has been reviewed and evaluated by the National Oversight Committee of the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR), and has been awarded AACVPR Program Certification for meeting all essential standards of care that are required for cardiac rehabilitation.
How To Find Us
The Cardiac Rehabilitation Center is located on the second floor of Our Lady of the Lake Medical Plaza II building. Enter the Medical Plaza II parking lot from Hennessy Boulevard. Parking is free for patients visiting the Medical Plazas.
