The Diabetes & Nutrition
Center is committed to providing optimal health and well being. We provide
two distinct, although complementary services (Diabetes Self-Management Training
and Medical Nutrition Therapy) for people with the diagnosis of diabetes or any
other nutrition related medical diagnosis (i.e. high cholesterol, high blood
pressure, weight management, etc.). Working closely with the referring
physician, our staff of certified diabetes educators - registered nurses and
registered dietitians, and exercise physiologists, as a team, provide training
and coaching to develop a treatment plan that fits your lifestyle.
The Diabetes & Nutrition Center has been
awarded Recognition by the American Diabetes Association in accordance with the
National Standards for quality Diabetes Patient Education Programs.
A Certified Diabetes Educator can help you:

Learn how to balance your eating, physical activity,
medication, and blood sugar monitoring routines.

Incorporate lifestyle needs, such as cultural eating
habits and exercise preferences, into your diabetes management plan.

Make traveling and dining out easier and healthier.
-
Learn how to prevent, recognize and treat high and low
blood sugars, and devise a plan for sick days.

Help prevent or delay other complications such as
heart disease, blindness, kidney failure, nerve damage, and sexual problems.

Learn prevention, early recognition, and treatment of
foot problems.

Decrease health care costs by reducing the need for
expensive hospital stays and the treatment of complications.

Develop healthy coping strategies and problem-solving
skills.

Improve your confidence in your ability to manage your
diabetes.
Our Services
Diabetes
Self-managment Training
Whether newly diagnosed or diagnosed for years, our team of
certified diabetes educators- registered nurses and registered dietitians- and
exercise physiologist provide education and ongoing support so that clients can
have a better understanding of how to live with diabetes.

and I learned how to live with diabetes from my diabetes
educator.'
Medical Nutrition Therapy (MNT) Outpatient
Service
Registered Dietitians are available to provide individual
MNT for people with nutrition related health problems, such as high blood
pressure, high cholesterol, weight management, kidney disease and
diabetes. Provides the opportunity to plan a nutrition program that
will allow long-term lifestyle changes to meet health goals.

Diabetes Education and Awareness Group In
Baton Rouge , monthly diabetes support group meets the second Tuesday
of each month in the Our Lady Of the Lake auditorium from 6 - 7
pm. In Gonzales , quarterly
diabetes support group meets the first Tuesday in the St. Elizabeth
Hospital auditorium from 6 - 7 pm.
Enrollment A referral from your physician will start your enrollment
for Diabetes Self-Management Training and/or Medical Nutrition Therapy.
Please call our staff for assistance in obtaining a physician referral. In
most cases, insurance covers the cost of these two programs. Our pre-cert
nurse will verify insurance benefits. Individuals are also
encouraged to call their individual health plan prior to enrolling.
Physicians may make outpatient referrals by calling
(225)765-8020 or by faxing written orders to (225) 765-6002. Inpatient
referrals are made by writing an order on the patient medical chart.
Referred individuals will be contacted by our staff to
arrange appointments. We offer individual and group appointments in order
to provide cost effective, high quality self-management training. Groups
are scheduled one full day or two half days.
The Diabetes & Nutrition Center team is available
Monday through Friday from 7:30 am to 4:00 pm.
